The parts were installed in error on US Model S vehicles during post-delivery service visits due to an error in the service parts catalog and parts ordering system that allowed ordering and consumption of the parts in the United States. Affected parts were identified based on Tesla Service records indicating such consumption. Because the condition was introduced by consumption of the parts in post-delivery service instead of production, the “production dates” above reflect the dates during which incorrect consumption occurred. The parts were installed in error on US Model S vehicles during post-delivery service visits due to an error in the service parts catalog and parts ordering system that allowed ordering and consumption of the parts in the United States. Production Dates : SEP 22, 2021 – DEC 31, 2021 Description of Noncompliance : Description of the Noncompliance : Affected driver’s side outside rearview mirrors are convex mirrors that do not comply with FMVSS 111, Section 5.2,1, which prohibits the driver’s side outside rearview mirrors from being convex. Affected passenger side outside rearview mirrors are convex mirrors that do not comply with FMVSS 111, Section 5.4.2, because they do not include the following warning etched on the mirror: “Objects in Mirror Are Closer Than They Appear.” FMVSS 1 : 111 – Rear visibility FMVSS 2 : NR Description of the Safety Risk : A mirror that does not comply with FMVSS 111 could cause a driver to misinterpret the mirror’s field of view, which could increase the risk of a collision. Description of the Cause : Side view mirrors designated for foreign markets were inadvertently made available to service teams ordering for U.S. market consumption, resulting in consumption during service visits of US vehicles. Identification of Any Warning that can Occur : The convex shape of the driver’s side mirror causes a reflection and field of view that is visibly discernable from the required flat mirror. The absence of the required warning text on the passenger side mirror is also visibly detectable. Involved Components : Part 573 Safety Recall Report 22E-092 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Component Name : EXT.MIRROR ASSY PREM EXP, LH MS P2 Component Description : SIDE VIEW MIRROR, LH Component Part Number : 1622232-00-B Component Name : EXT.MIRROR ASSY PREM EXP, RH MS P2 Component Description : SIDE VIEW MIRROR, RH Component Part Number : 1622233-00-B Supplier Identification : Component Manufacturer Name : NR Address : NR NR Country : NR Chronology : – In August 2022, Tesla was made aware of a forum post in which a customer identified receiving a side view mirror for European market consumption during a service visit in the United States for his vehicle, which was manufactured for, delivered and operated in the United States. – From August 10 through October 6, 2022, Tesla investigated service parts ordering histories and distribution of the suspect part numbers within the service channels. – From October 6 through October 21, 2022, Tesla investigated and determined consumption of 53 of the suspect part numbers during service visits of US vehicles. – On October 25, 2022, Tesla determined that the EU side mirrors that were consumed in the US market during service visits were not compliant with FMVSS 111. Accordingly, Tesla made a determination to voluntarily recall the mirrors. – As of October 25, 2022, Tesla is not aware of any warranty claims, field reports, crashes, injuries, or deaths related to this condition. Part 573 Safety Recall Report 22E-092 Page 4 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : At no cost to the customer, Tesla will replace the recalled mirrors with mirrors that comply with FMVSS 111. Customers who paid to purchase either of the recalled mirrors may be eligible for reimbursement per Tesla’s General Recall Reimbursement Plan. How Remedy Component Differs from Recalled Component : The remedy part for the recalled driver’s side outside rearview mirror is not convex and complies with FMVSS 111. The remedy part for the passenger side outside rearview mirror is convex and complies with FMVSS 111, including the following warning etched on the mirror: “Objects in Mirror Are Closer Than They Appear.” Identify How/When Recall Condition was Corrected in Production : No correction is needed in production. However, the Tesla Service parts catalog was updated, and additional measures were implemented in the parts ordering system to prevent U.S. service teams from ordering and consuming outside rearview mirrors that do not comply with FMVSS 111. Recall Schedule : Description of Recall Schedule : All Tesla stores and service centers will be notified about this recall on or shortly after November 3, 2022. Owner notification letters will be mailed in accordance with 49 C.F.R. § 577.7.
