Recall Bulletin: Recall Bulletins are mandatory service procedures that must be carried out by Tesla-certified Service Centers. Recall work performed by uncertified technicians could lead to unsafe conditions or voided warranty provisions. Condition On certain vehicles operating firmware releases before 2022.4.5 (except 2021.43.101.1), under certain circumstances, the driver seat belt reminder chimes may fail to reset after sounding a reminder, and may not sound upon the next vehicle start cycle. The seat belt chimes will continue to sound when the vehicle exceeds 14 mph (22 km/h) with the driver seat belt unbuckled, and the visual seat belt reminder is not affected in any way. Correction Inspect the firmware version installed in the vehicle, and if necessary, update to version 2022.4.5 or later. On the vehicle touchscreen, touch Controls (vehicle icon) > Software, and then scroll down to display the vehicle firmware version. • If the vehicle is operating with firmware version 2022.4.5 or a newer release, then no further action is necessary. Use correction code S012200002. • If the vehicle is operating with firmware version 2021.43.101.1, then no further action is necessary. Use correction code S012200002. • If the vehicle is operating with a firmware version before 2022.4.5, and it is not 2021.43.101.1, then update to the latest available firmware release. Tesla, Inc. has decided that certain Model Year 2021-2022 Model S and Model X, 2017-2022 Model 3 and 2020-2022 Model Y vehicles fail to conform to Federal Motor Vehicle Safety Standard (“FMVSS”) No. 208 “Occupant Crash Protection.” Our records show that you are the owner of a vehicle affected by this action. REASON FOR THIS RECALL Under certain circumstances, a software error may prevent an audible seat belt reminder chime from activating upon vehicle start as required by FMVSS No. 208. This condition is limited to circumstances where the chime was interrupted in the preceding drive cycle and the seat belt was not buckled subsequent to that interruption. The visual seatbelt reminder on the touchscreen remains unaffected. If the audible seat belt reminder chime does not activate at the start of a new drive cycle and the driver does not notice the accompanying visual seat belt reminder, the driver may not be reminded to buckle their seat belt and may begin operating the vehicle in an unbuckled state, which could increase the risk of injury. WHAT TESLA WILL DO Tesla remedied the noncompliance with a free over-the-air (“OTA”) software update that corrects the software error. WHAT YOU SHOULD DO Please check that your vehicle is running software version 2021.43.101.1, 2022.4.5, or any later release, all of which correct the software error. You can check the software version running on your vehicle by tapping ‘Controls’ then ‘Software’ on your touchscreen. If your vehicle is running any one of the software versions listed above, then there is no further action that you need to take, and you may disregard this notice. If your vehicle is not running one of these software versions, then please install the latest pending software update through your Tesla app, or by tapping the yellow clock icon on your vehicle’s touchscreen and following the prompts. Production Dates : OCT 09, 2019 – JAN 30, 2022 VIN Range 1 : Begin : NR End : NR Not sequential Description of Noncompliance : Description of the Noncompliance : FMVSS 208, S7.3 (a)-(1), requires the audible seat belt reminder chime to activate upon vehicle start (i.e., driver presses the brake pedal after entering the vehicle) if the driver seat belt is not detected as buckled. On certain MY 2021-2022 Model S and Model X vehicles and on all MY Model 3 and Model Y vehicles, a software error may prevent the chime from activating upon vehicle start under certain circumstances. This condition is limited to circumstances where the chime was interrupted in the preceding drive cycle and the seat belt was not buckled subsequent to that interruption (e.g., the driver exited the vehicle in the preceding drive cycle while the chime was active and later returned to the vehicle, creating a new drive cycle). This condition does not affect the audible seat belt reminder chime from activating when the vehicle exceeds 22 km/h and the driver seat belt is not detected as buckled. The condition also does not affect the reliability and accuracy of the accompanying visual seat belt reminder at any point. FMVSS 1 : 208 – Occupant crash protection FMVSS 2 : NR Description of the Safety Risk : If the audible seat belt reminder chime does not activate at the start of a new drive cycle and the driver does not notice the accompanying visual seat belt telltale, the driver may not be reminded to buckle their seat belt and may begin operating the vehicle in an unbuckled state, which could increase the risk of injury. Tesla is not aware of any injuries or fatalities related to this condition. Note that when the vehicle exceeds 22 km/h and the driver seat belt is not detected as buckled, the audible seat belt reminder chime will activate and remind the driver to buckle their seatbelt. Description of the Cause : When the driver starts the vehicle without buckling their seat belt, software monitors the current state and warns the driver to buckle their seat belt, and records that it has warned the driver. However, if the drive cycle state changes (e.g., driver exits the vehicle) during this reminder, the software erroneously stays in the state where it has already warned the driver, and does not reset its state to prepare for the next warning. Before 2022.4.5 exc. 2021.43.101.1 Component Description : NR Component Part Number : NR Supplier Identification : Component Manufacturer Name : Tesla Address : 13101 Tesla Road Austin Texas 78725 Country : United States Chronology : – On January 6, 2022, the South Korea Automobile Testing & Research Institute (KATRI) brought the condition and limited circumstances to Tesla’s attention. – From January 10, 2022 to January 22, 2022, the Tesla vehicle software and homologation teams investigated the condition and scope of vehicles that were potentially impacted by it. – On January 25, 2022, Tesla reviewed the investigative findings and determined that the condition is a noncompliance. A recall determination was voluntarily made the same day. – As of January 31, 2022, Tesla is not aware of any warranty claims, field reports, crashes, injuries or fatalities related to the condition. Part 573 Safety Recall Report 22V-045 Page 4 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : A firmware release will correct the software error, so that the audible seat belt reminder chime will reset if it is interrupted while chiming. Firmware release 2021.43.101.1, which includes this remedy, was introduced in Model 3 and Model Y production on January 27, 2022, and in Model S and Model X production on January 28, 2022. Separately, firmware release 2022.4.5, which also includes this remedy, will deploy over-the-air (“OTA”) to delivered vehicles in early February 2022. No further action is necessary from owners whose vehicles are equipped with firmware release 2021.43.101.1 or 2022.4.5 or a later release. New vehicles will not be delivered to customers without 2021.43.101.1 or 2022.4.5 or a later release. Tesla does not plan to include a statement in the Part 577 owner notification about pre-notice reimbursement to owners because there is no paid repair relating to the underlying condition and owners will receive the remedy free of charge with an OTA firmware release. How Remedy Component Differs from Recalled Component : Firmware release 2021.43.101.1 or 2022.4.5 or a later release will correct the software error, so that the audible seat belt reminder chime will reset if it is interrupted while chiming. Identify How/When Recall Condition was Corrected in Production : Firmware release 2021.43.101.1, which includes the remedy, was introduced in Model 3 and Model Y production on January 27, 2022, and in Model S and Model X production on January 28, 2022.
